Insurance & Payment Options
Accepted Insurance Plans: We accept most major insurance plans. Please contact our office at (314) 350-8207 to confirm if we are in-network with your plan. We also recommend you contact your insurer directly to confirm we are in-network with your plan.
Copays & Deductibles: Copays, coinsurance, and any unmet deductibles are due at the time of service.
Out-of-Network & Self-Pay Patients: If we are out-of-network with your insurance, or if you are uninsured, we offer transparent pricing and flexible self-pay options.
Credit Card on File Policy
To streamline the billing process and reduce administrative costs, we require all patients to keep a credit or debit card securely on file.
Your card will not be charged without prior notice, except for the following:
- Outstanding balances not paid within 60 days of your statement date.
- No-show or late cancellation fees as outlined in our policy.You will receive an itemized statement before any charges are processed, and we will always attempt to contact you regarding any unpaid balances before charging your card.
Your payment information is stored securely and complies with all HIPAA and PCI standards.
Cosmetic & Elective Procedures
Cosmetic treatments (e.g., Botox, fillers, laser procedures, skin rejuvenation) are not covered by insurance. Payment for these services is due in full at the time of your appointment.
Package Pricing & Promotions: We offer discounted package pricing for select cosmetic treatments. Ask our team about available specials!
Pathology & Laboratory Fees
Separate Billing for Lab Services: If a biopsy, culture, or other lab test is performed during your visit, the sample may be sent to an outside pathology or laboratory service for processing.
Billing Information: You will receive a separate bill from the pathology lab or testing facility. We do not control the billing of these external labs, so please contact them directly if you have questions regarding your statement.
Billing & Payment Policies
Payment Methods Accepted: We accept cash, credit/debit cards (Visa, MasterCard, American Express, Discover), HSA/FSA cards, and personal checks with proper identification (valid Driver’s license or photo ID).
Outstanding Balances: Any outstanding balance must be paid before scheduling additional appointments. Flexible payment arrangements may be available upon request.
Prior Authorization & Prescription Costs
Some medications and procedures may require prior authorization from your insurance company. Our team will assist with this process, but final coverage decisions are made by your insurer.
Patients are responsible for any pharmacy copays or costs not covered by insurance. We recommend checking with your pharmacy and insurance provider regarding medication pricing.
Requests for Medical Records
Patient Portal Access: Our practice uses an electronic medical record system with a secure online patient portal, where you can access your medical information at no charge.
Printed Medical Records: If you need a printed copy of your medical records, a fee of $0.10 per page plus postage will apply.
Transferring Records: We will send medical records to another healthcare provider at no charge upon request.
